1) Yes, Lr does create the folder "Lightroom" in <My Pictures> But that can be changed and it should be changed because it is precisely this default that brought you into this predicament.
So, please re-read my first post regarding the top-level folder for the catalog.
2) Having the "Backup folder" in the Lightroom folder is another default of Lr but this is just another mistake. You need to have the Backup catalog in a different location than your "working" catalog. When you close Lr and the backup dialog comes up, select a different location for the backup, preferably on an external hard drive.
3) You don't have to accept what Lr offers you for backup catalog. With Lr closed just double-click the one catalog backup that you think is the latest, and Lr will open with it. If it is not the right one, close Lr and double-click another backup catalog, until you find the one that is the latest.
